NEW HIRE FORMS

All enrollments/declination of insurance for new hires must now be done online through the employee’s web benefits portal.  An enrollment email will be sent to the employee once they’ve been entered into the payroll system but no more than 2 weeks prior to their insurance effective date.

Employee Emergency Notification Form

Federal W4 2023 – to be used only for a new hire not doing online onboarding; current employees are required to change their withholding online in their Paylocity self-service portal.

WH-4 Indiana State Form -to be used only for a new hire not doing online onboarding; current employees are required to change their withholding online in their Paylocity self-service portal.

Direct deposit – to be used only for a new hire not doing online onboarding; current employees are required to change their withholding online in their Paylocity self-service portal.
