Free Preventive Benefits

When you elect our high deductible medical plan, you and your covered family members are eligible for many FREE preventive services.

• Routine exams including routine diagnostic test, lab and x-rays
• Well baby care
• Routine pelvic exams and pap tests
• Routine mammograms
• Routine PSA test (prostate screening)
• Immunizations (flu shots, tetanus shots, etc.)
• Prescription drugs for smoking cessation up to $1,000 per year
• Prescription drugs for weight loss up to $1,000 per year

Preventive services do not apply toward your deductible, co-insurance or out-of-pocket maximum.


VISION – AUTOMATIC COVERAGE FOR MEDICAL PLAN PARTICIPANTS

If you elect the health plan, you and your covered family members are eligible for an annual eye exam free of charge from an in-network provider.  We do not have a separate vision plan.  The only vision expense covered is the one free eye exam and it is covered and billed under the preventive portion of your medical plan.  It is important to note that this benefit will not cover any other type of procedure other than the exam (which can be as basic as reading an eye chart).
 

The United Health Plan (effective 1/1/18)

How to find an eye doctor for the annual eye exam covered under The United Health Plan:

  • Go to www.umr.com
  • Click on “find a provider”
  • In the provider network text box enter UnitedHealthcare Choice Plus then hit the search button
  • Select the “change location” link at the top and enter the zip code you would like to find an optometrist
  • In the search field enter “optometry” and then select the search button (make sure you specify a zip code)
  • This will pull up a list of in-network providers

When you call your provider to make your annual exam appointment, tell them specifically that your annual eye exam is covered as a “routine eye exam under the preventive care benefit of our medical plan”.  Upon your visit, provide the office with your medical ID card.


REMINDER!

You can pay for eye care including eyeglasses, contact lenses and corrective eye surgeries (LASIK or PRK) using your Health Savings Account (or your Flexible Spending Account or Health Reimbursement Account for those not electing the HSA).
