Continuation of coverage – post employment and for overage dependents

Continuing Coverage

If your employment with the Archdiocese ends and you are covered under our high deductible medical plan, you may elect to continue your coverage for up to 18 months.

If you are age 60 or over and have worked for us for 10 years or more, you can continue your coverage for up to five years.  Eligibility for continuation ceases when employee becomes eligible for Medicare.

If you wish to continue your coverage, you must contact Human Resources within 30 days of the termination of your employment.  Information regarding continuation of coverage is only mailed out to the employee upon request by the employee.

Over Age Dependents

If your children are covered by our medical or dental plans, remember that their coverage ends on the last day of the month in which they turn 26 – regardless of whether or not they’re a full time student. If your child becomes ineligible for coverage, the child may purchase continuation coverage under our plan for up to 18 months.

You must request continuation by contacting the Human Resources department.  Information is not mailed out upon termination from the plan.  It’s only mailed out upon request by the employee.
